CHICAGO HEALTH DISPARITIES CENTER

The Â鶹APP Health Disparities Center (CHDC) is a research group funded through the National Institute on Minority Health and Health Disparities (NIMHD) and Patient Centered Outcomes Research Institute (PCORI). Located at Illinois Tech and directed by Patrick Corrigan and Lindsay Sheehan, CHDC is committed to reducing physical health disparities for racial and ethnic minorities with mental illness. CHDC is partnered with Â鶹APP-area universities, service provider organizations, and public partners, including the Â鶹APP Department of Public Health.

COMMUNITY COUNSELING CENTERS OF CHICAGO

Community Counseling Centers of Â鶹APP (C4), a behavioral health advocate and social service provider, offers quality, comprehensive customer-oriented services tailored to the diversity of its consumers. C4 believes that people recovering from mental illness and emotional trauma are able to live, work and thrive in the community. Treatment is viewed as a partnership with the individual, C4 staff, and when possible, a network of family and friends. C4 offers a comprehensive range of services designed to help adults, children and families cope with varied behavioral health challenges.

NATIONAL CONSORTIUM ON STIGMA AND EMPOWERMENT

The National Consortium on Stigma and Empowerment is a research group meant to promote recovery by understanding stigma and promoting empowerment. Located at the Illinois Institute of Technology and directed by Patrick Corrigan, the consortium works with a collection of researchers at Yale University, the University of Pennsylvania, Rutgers University, Temple University, the University of Wisconsin, Illinois State University, and the University of Â鶹APP.

THRESHOLDS

Thresholds provides healthcare, housing, and hope for thousands of persons with mental illnesses and substance use disorders in Illinois each year. Through care, employment, advocacy, and housing, Thresholds assists and inspires people with mental illnesses to reclaim their lives.

Thresholds is one of the oldest and largest providers of recovery services for persons with mental illnesses and substance use disorders in Illinois. The organization offers 30 innovative programs at more than 100 locations throughout Â鶹APP, the adjacent suburbs, and nine surrounding counties. Services include assertive outreach, case management, housing, employment, education, psychiatry, primary care, substance use treatment, and research.

SHIRLEY RYAN ABILITYLAB

The Shirly Ryan AbilityLab (formerly the Rehabilitation Institute of Â鶹APP) conducts comprehensive interviews with clients to review pertinent medical, educational and employment information in order to develop personalized vocational rehabilitation service plans that meets their needs.

SOUTH CAMPUS

South Campus (SC) is a private therapeutic day school in Palatine, Illinois, that serves students in Southern Lake and Northern Cook Counties who are in need of an alternative school program that is supportive, therapeutic and educational. The program serves students between the ages of 6 and 21 years. SC utilizes educational, therapeutic, family and community connections to promote academic achievement and personal/emotional growth.

Trilogy, Inc., is a private not-for-profit behavioral healthcare organization with more than 45 years of experience serving people with serious mental illness in the City of Â鶹APP and its surrounding suburbs. Trilogy's mission is to support people in their recovery from mental illness by helping them discover and reclaim their capabilities, life direction and well-being. Trilogy is a Trauma-Informed Care organization with an appreciation for the high prevalence of traumatic experiences in persons who receive mental health services. Every single client served by the organization experiences a safe and trusting physical and social environment that provides trauma-informed and trauma-specific services that are easily accessible; person-centered, competently delivered, culturally respectful and consistent with recovery-oriented principles of collaboration: shared decision making and self-direction.
